Financial & Payment Policy
The Neurosurgical and Spine Institute is committed to providing you with the best possible care regardless of your coverage. Below is a summary of our financial and payment policies.
- As a courtesy to you, we will file your insurance claim form for reimbursement. However, it is the patient’s responsibility to confirm that current insurance information is on file.
- Many insurance carriers require pre-authorization for specialized and surgical procedures and/or referrals from a primary care physician. If you are unable to obtain the authorization, you can sign a medical waiver and pay us directly for the services we provide you, and we will refund you when we receive the proper authorization for those services.
- If you have not met your deductible for the year, you will be asked to pay in full at the time services are provided until your deductible is met.
- After you have met your deductible, you will be asked to pay your co-pay or co-insurance amount at the time services are provided.
- Surgical procedures will require a deposit at the time of scheduling you for your procedure.
- Charges not paid by your insurance company within 45-60 days will become due and payable by you.
- We accept MasterCard, Visa, American Express and Discover credit cards, as well as personal checks and cash.
- Refunds will be issued if overpayment occurs.
- Please be aware that any patient balance on your account over 90 days is subject to collection procedures and may result in denial of future care until arrangements are made to resolve any overdue balances.
- Parents, a designated family member, or legal guardian are responsible for payment for those services rendered to children in their care.
- We will bill for Workers’ Compensation services that have been authorized by your employer or Workers’ Compensation insurance carrier.
- Please be aware our office charges a $25 fee for missed appointments.
- We also additional fees for services outside your visit including completion of FMLA/disability forms, medical records review, medical depositions and additional copies of images on CD.

If your plan is not listed above, please check your PPO/commercial plan benefits to see if our surgeons are listed as an in-network provider. If not, we will also be glad to assist you by providing the information needed by your plan to reimburse you under out-of-network benefits.
The Neurosurgical & Spine Institute also welcomes patients without health insurance and offers affordable rates along with a discount when paid in full at the time of service. Payment methods currently accepted include Visa, MasterCard, Discover, American Express, check and cash.
Paying for Treatment
Our mission is to provide you with high-quality health care that you can afford. If insurance does not cover the entire cost of your treatment and you need to make payments for the medical care you receive, we will work with you. We accept all major credit cards and offer convenient payment options, including in-house plans. If you do not have insurance, we offer discounted rates to those who are self-pay.
